House Officers Long Term Disability Insurance Program

Coverage While in Training - Group Long Term Disability (LTD) Highlights:
ü Monthly benefit is 66 2/3% of monthly salary (maximum $4,000 per month)
ü Benefit Paid to Age 65
ü Resident pays no premium for this coverage
ü No enrollment necessary; automatic with entry into training
LTD Plan Certificate
Coverage at Completion of Training - Individual Disability Conversion Highlights:
ü Guaranteed Issue - Up to $12,500 per month without evidence of medical insurability
ü Own Specialty definition of disability
ü No limitation for disabilities due to Mental Disorders or Substance Abuse (MDSA)
ü Level premiums guaranteed
ü HOA discounts and unisex rates apply for the life of the policy
